Children's Case Management
We offer case management services to help youth and their families arrange the therapeutic supports they need to function effectively in community settings.
Our case managers work closely with parents and young people to develop a plan of care that fits the family's needs. Coordinated services may include clinical assessment, therapy, psychiatric services, and other supportive interventions.
The goal of children's case management is to intervene early enough to promote healthy social and emotional experiences that can help to prevent challenges later in life. We value the parent's role in advocating for the most effective treatment available.

Family Support Services (HCT)
Community Family Support Services (CFSS) is a home-based, family-centered service also known by the Department of Health and Human Services as Home and Community Treatment (HCT). CFSS assists families in finding solutions to a variety of stressors that occur primarily in the home environment. The CFSS program is designed to be intensive and short-term, with families generally completing treatment in 3-9 months.

Mental Health Clinic
Our comfortable and relaxed environment at Community Care is designed to put visitors at ease. Our office is a place where seeking mental health support is respected, and our professional staff has an unconditional positive regard for those in need of services.
If you or your family is seeking help with relationship problems, mental health issues, or substance abuse challenges, our outpatient clinic can provide you with individual, family, and substance abuse therapy.
We work closely with you and other helpers to figure out how much support you want and need, and then we build a plan to fit your individual or family situation. We believe that your active participation in your own plan of care is the key to helping you cope effectively with your challenges and feel hopeful about your future.

Treatment Foster Care
Treatment foster care offers high needs youth structured, stable, and temporary family living environments as they progress toward reunification, kinship care, adoption, guardianship, or independent living. Treatment foster parents are professionals who are specially trained to repond therapeutically to the demands associated with caring for young people who are in varying stages of recovery from challenging early life experiences.
